MAOT Membership Application (pdf format)

   Using the pdf format, you can enter your information right on the form.  When you have finished, simply use the PRINT command in your browser toolbar to print a copy and mail it, along with your check or money order, to the address shown on the form.  After you have printed the form, click on your BACK button to go back to the website.  Or, simply click on the Pay using credit card/PayPal - click here link on the form and pay right online.
